Parental Consent/Medical Release forms are required for student ministry events. We ask each student to fill out a consent form each calendar year that will be put on file. Once a form is turned in, no other form is necessary until the following year (unless a form is required for an event led by an outside organization). We do not wish to overburden parents by filling out a form for every activity that takes place.
